No flames here. I'd just like to point out that such absolute statements may invite them though... ("...which stands as the best discussion of O/R-M in one volume to date.") :-)

Anywhoo, what about the new-ish, so-called "Space Based Architechtures"? Yes, they need a specific context to be beneficial, but they seem to have several aspects which may alleviate some of the pains you outlined. From the "middle tier" it seems like you are talking to an OODBMS, but in the end, you actually write out to a RDBMS (or any other kind of storage...).
